• Graduate Faculty Membership Nomination Form (pdf) (for Full or Associate Status)
  Upon Hire - complete and submit nomination form to the Graduate School as soon as hiring has been completed and all needed documents have been submitted to the Academic Personnel Office.
  
  For Current Faculty - complete and submit nomination form to the Graduate School either for fall round (November 1 deadline) or spring round (March 1 deadline).
